CERTIFICATION/LICENSURE:

- Masters of Science or Doctorate in Physical Therapy from an accredited programn
- Successful completion of National Certification Examinationn
- 1 year of verifiable, supervised professional experience within the last 3 yearsn
- Valid PT license in state of practicen
